Throughout her career as a care manager in the past since 2013, Wendy has assisted hundreds of individuals and families who are struggling with caregiver distress. In recent years Wendy and her team have done excellent work in the area of crisis management. When we are called upon for hire as care manager/aging life care specialists we must jump in immediately to do an assessment of the elder care crisis in that moment. We will suggest interventions, resources and approach the crisis at hand.
We have managed many Covid health care crisis since 2020, elderly housing crisis, need for immediate hospital and rehabilitation crisis and other related matters that need a quick response team approach.
We have addressed palliative care and hospice referrals and other related end of life matters with grace and patience. We continue to do so as part of our GCM practice.
We have been hired for staff trainings and other speaking engagements . Wendy has created a large Sabin Elder Care Networking group that meets several times per year. We are hosted by many facilities and care teams. These groups have grown to include over 300 elder care experts in New Jersey.
In October, 2013 Wendy Sabin launched Sabin Elder Care Solutions, a private geriatric care management practice based out of Montclair, NJ. Since the launch of her practice, Wendy has received numerous responses, referrals, connections and outreach from the greater Elder Care Community in this part of New Jersey. According to Wendy, “the support and attention has been most exciting and the growing need for guidance in this elder care field has only reinforced the importance of having a care manager and advocate along the way!”
Wendy Sabin, MSW, LCSW received her Master’s Degree in Social Work from New York University in 1989. She spent the first years of her career working as a clinical social worker in NYC in the Employee Assistance Program field. It was there that she began to do short term, solution-focused therapy, working with mental health, substance abuse, family crisis, care giver and other clinical issues. In more recent years, Wendy worked for Jewish Family Services in NJ as a Synagogue Social Worker in 2 New Jersey synagogues in Essex County.
Her work there focused on crisis intervention, geriatric and care giver issues, parenting/family matters, employment and other personal issues. Wendy developed and ran multiple groups and programs. But one of the main functions of her role over the years was to plan “care giver support groups”. The groups were very effective and necessary when dealing with “sandwich generation adults”. While working at the synagogues, Wendy also worked within two local senior centers in Verona and Caldwell, NJ.
While working in these two NORC (Naturally Occurring Retirement Communities) which were funded by a state grant, Wendy recognized the serious need for professional support services for seniors and their family members who are aging in place. During much of her time with JFS, the focus of her eldercare work was to act as a guide/ advocate for older adults and their families who were struggling with significant, personal issues. Wendy is also a Certified dementia practitioner.
Dawn Paglia Berman, MSW, has extensive geriatric, medical and clinical social work experience. Prior to joining Sabin Elder Care Solutions, where she works with aging adults and their families to ensure they maintain a high quality of life and receive superior care, Dawn was a case manager, program director, and Licensed Nursing Home Administrator at ManorCare Health Services. Dawn also served as regional director at Multicare Healthcare Services, overseeing 17 skilled nursing and sub-acute rehabilitation facilities throughout New Jersey.
Dawn began her career working as a medical and clinical social worker at several New Jersey medical centers including Beth Israel, St. Michael’s, and St. James. Dawn also worked in the area of brain injury rehabilitation at the nationally recognized Kessler Institute for Rehabilitation.
Dawn holds a MSW from Rutgers University.
